Your SPN 5246 is the speed derate for the the other 2 faults. In my experience, clogged doser valves are the #1 cause for SCR problems. Failed NOx sensors and NH3 sensors are common as well, but have their own fault codes. The first thing I would do id pull the DEF doser off and run the dosing test to see the spray pattern and measure the output.
