How do tire caps fall off?

The caps have an o-ring in then that seats and helps seal the valve from dirt. Pressure-wise on the high side you’re looking at over 300 psi. On the low side 60-90 psi IIRC. The seal in the cap is useless to hold that back. If the system was just checked and charged, you might want to tell them and see if they will fix it.

Can a low pressure port cap prevent a refrigerant leak?
The cap doesn’t do anything to prevent a refrigerant leak, but I wouldn’t drive around without it as it does keep keep dust/grit/water out of the port which can cause other issues and complicate future service. Cover the port with something. Almost anything will do, even a big piece of heat shrink with the end folded over etc.
