Can you teleport out of I AM ERROR room?
The Error Room in The Womb/Utero 2 will only have a trap door to Sheol, and not a beam of light to the Cathedral. You cannot be teleported to the I AM ERROR room on Sheol or the Cathedral.

The I AM ERROR room is a reference to an infamous quote in the NES game Zelda II: The Adventure of Link, where a citizen from Ruto introduces himself with the sentence “I AM ERROR”. Due to the relative difficulty involved in accessing this room, and the name of the room itself, it could be a failsafe.
Why do I keep teleporting in binding of Isaac?
The teleport effect only occurs when taking damage while in the middle of charging, before the shot is fully charged. Having a high tears stat for faster charging will decrease the window where Isaac is vulnerable to teleporting.

Why does Mega Satan go into his second phase?
Using Chaos Card on the first phase causes Mega Satan to go into his second phase instead of instantly killing him. This is because the first and second phase of the fight are actually two different enemies, and are shown as such in the bestiary.
