How do you get Nashandra to spawn?
To access Nashandra, you need to have defeated Throne Watcher and Throne Defender and obtained the Giant’s Kinship and the King’s Ring.
How do I get to the final boss in Dark Souls 2?
In order to reach this place, travel to Drangleic Castle’s “King’s Gate” bonfire. Leave the bonfire room and turn left, equip the King’s Ring and the door will open. Go inside and follow the road. After entering the fog gate the first boss fight will start.

Behind a locked door you need to open with the Soldier Key near the end of the Forest of Fallen Giants. From the second bonfire, continue til you reach the raised stone arena. This is a one-time encounter and he will not re-appear if you are defeated or leave the arena.

He will appear to the player at the the last Primal Bonfire they reach, the entrance to the Undead Crypt and the Dragon Shrine. If spoken to in all of these locations he will become the final boss after defeating Nashandra at the Throne of Want.
What happened to Nashandra?
Knowing that his time was short and that his queen had to be stopped, Vendrick created several impasses that would prevent Nashandra from seizing the throne, and sealed himself away in the Undead Crypt. This left Nashandra the sole ruler of Drangleic, albeit without much left to rule at all.
