When do you give Digoxin Immune Fab?
Digoxin immune Fab is indicated for the treatment of life threatening digoxin toxicity in patients with manifestations of severe toxicity. It is NOT indicated for mild cases of digoxin toxicity or in cases of elevated serum digoxin concentrations in the absence of signs and symptoms of digoxin toxicity.
What is the use of Digoxin Immune Fab?
Digoxin immune FAB is used as an antidote to treat a life-threatening overdose of digoxin or digitoxin. Digoxin immune FAB is not for treating a mild digitalis overdose. Digoxin immune FAB may also be used for purposes not listed in this medication guide.
Is Digoxin Immune Fab the same as digoxin?
Digoxin immune fab or digoxin-specific antibody is an antidote for overdose of digoxin. It is made from immunoglobulin fragments from sheep that have already been immunized with a digoxin derivative, digoxindicarboxymethoxylamine (DDMA). Its brand names include Digibind (GlaxoSmithKline) and DigiFab (BTG plc).

For both brands of digoxin immune Fab, one vial of the product will bind approximately 0.5 mg of digoxin. Therefore, the dose of digoxin immune Fab is based on the amount of excess digoxin believed to be present in the patient experiencing toxicity.
When do you give DigiFab?
Give DigiFab® by bolus injection, if cardiac arrest is imminent. An increased incidence of infusion-related reactions may be expected with bolus injection. For infants and small children who may require very small doses, reconstitute the 40 mg vial as directed and administer undiluted using a tuberculin syringe.

DigiFab is indicated for the treatment of patients with life-threatening or potentially life- threatening digoxin toxicity or overdose. Although designed specifically to treat digoxin overdose, a product very similar to DigiFab (Digibind) has been used successfully to treat life-threatening digitoxin overdose7.
How do you know if Digoxin Immune Fab is effective?
Digitalis-induced progressive elevation of the serum potassium concentration also suggests imminent cardiac arrest. If the potassium concentration exceeds 5 mEq/L in the setting of severe digitalis intoxication, therapy with DIGIBIND (digoxin immune fab) is indicated.
How is DigiFab given?
DigiFab should be administered slowly as an intravenous infusion over at least 30 minutes. If infusion rate-related reactions occur, the infusion should be stopped and re-started at a slower rate. If cardiac arrest is imminent, DigiFab can be given by bolus injection.
