Why does my car sometimes shift hard?

While the car is on ‘D’, or ‘Drive’, and I am pressing on the accelerator and trying to shift over 30mph, it feels as though the car is automatically reverting into neutral. I say this because you can clearly hear the engine revving up, but the car is not moving any faster.
