What was the cause of the timing belt failure?

Finally got FTTN, 710m line length…. catastrophic failure of the engine, all the valves are bent, pistons are all scored, the pulley that failed exploded in the case destroying the timing belt, tensioner and all associated bits, timing belt didn’t snap but during the failure but the timing changed and this caused the pistons to hit the valves.
