Why does my Mazda B series not shift?

At times 1st gear just keeps revving and doesn’t seem to want to shift and when it does there is quite a jolt when it does due to the thunking. Almost feels like someone has jammed the back of the truck. I have had it checked by the Madza dealer who greased/oiled/and checked all fluids etc, and THEN took it for a test drive.
