Does white smoke mean running lean?
Just becuase something has not happened to you, does not mean, it does not happen at all. Excess fuel burns black, always has. Oil burns blueish and white is water/coolant. As for running lean with white smoke, never heard of such a thing unless it’s water/coolant seeping through the headgasket.
Can running too rich cause white smoke?
A diesel engine requires precision timing and fuel pressure of the injector pump. When the timing is not what it’s supposed to be, your engine will essentially be running rich which will cause fuel to not completely burn and instead exit out of the exhaust as white or gray smoke.

One other problem, maybe the timing advance is slipping internally causing the timing to not advance properly. This should be chacked by someone who knows to to check, repair and retime the engine. Having lots of black smoke and high idle is usually a good indicator that you have a leak. Replace the boots you have coming and time the motor.
What does blowby smoke in an engine mean?
BLOWBY (or BLOWBY SMOKE) is the emission of crankcase pressure (and fumes/smoke) out the crankcase ventilation system, and generally regarded as a clear indication of an urgently needed, and expensive, engine rebuild. It is evident as pressure coming out the crankcase ventilation tube, valve cover breather or dipstick tube.
Can a diesel engine burn oil without blue smoke?
Blue smoke should not be evident at any time, but it is worth noting, that engines with good sound compression can actually burn quite a lot of oil without evidence of blue smoke. Good compression allows oil to burn cleanly, as part of the fuel. It is not good though!
